Ratio:

• Change detail namespace by ratio_detail
• Move helper to specific file
• Move general use metafunctions to StaticInteger lib
• Prefix intmax_t by boost::
• Define the variable ABS_N and ABS_D before asserting
• Provide ratio constructor and assignment from an equivalent ration conditionally
• Make ratio arithmetic operation inherit from ratio
• Remove #if 0
• Make public ratio_gcd as used by Boost.Chrono
