1884	make_shared.hpp and test	Frank Mori Hess	Peter Dimov	I'm attaching boost/make_shared.hpp and libs/smart_ptr/test/make_shared_test.cpp.  The files are basically the reference code make_shared.cpp split into two files, with some namespaces added.  I wasn't sure how to handle the check for the __STRICT_ANSI__ macro, so I just left it in.  The test passes on my local machine (gcc 4.1.2).
