If the basic_io_object constructor is being interrupted by an exception, it will be creating an access-violation within the destructor due to the fact, that te reference will not have been initialised.
For my local installation, I added a bool variable, tracking the initialisation and throw an exception in the get* functions, and do nothing within the destructor .
