posix_recursive_mutex constructor never called, fails to init mutex

I'm trying to get a shared memory system working on OSX, as far as I can tell, it's calling posix_recursive_mutex::lock() without initalizing the mutex, which happens in the constructor. A breakpoint on line 78 of include/boost/interprocess/sync/posix/recursive_mutex.hpp never gets hit.
