Can't set or unset serial hardware flow control on QNX

Description
serial_port_base::flow_control doesn't support hardware flow control on QNX. On QNX, the IHFLOW and OHFLOW flags are used to control hardware flow control for input and output respectively.

I've attached a patch to add this functionality. I've tested all the code paths on QNX 6.5.0 against the output of the stty command.
If the input and output flags are inconsistent I did what the Windows version seems to do, ie consider hardware flow control to be off.
